Register as a Vendor
When you register as a vendor, it means that you can do business with the SMHA. If you are interested in doing business with the SMHA, please follow the instructions below. If you have questions, you can call 937-325-7331 x204 or email procurement@smhaohio.gov.
How to Register as a Vendor:
- Complete Vendor Registration Packet Adobe fillable format
- SMHA Vendor Direct Deposit Authorization Agreement Adobe fillable format
- Thoroughly review the procurement guidelines
- Complete W9 form https://www.irs.gov/forms-pubs/about-form-w-9
- Provide BWC Certificate, insurance, & liability information if service vendor
- Email all documents to procurement@smhaohio.gov
Our purchasing procedure is based on a purchase order system only and no order should be accepted by your company without a purchase order number. That number must accompany each transaction concerning that order including packing slips, all packages, and invoices.
